By accessing our website or engaging our services, you agree to be bound by these Terms. If you do not agree, please do not use our website or services.
2. Description of Services
LabMed Billing provides laboratory-focused billing support services, including but not limited to:
Laboratory credentialing and payer enrollment
Laboratory coding (CPT, ICD-10, and HCPCS)
Denial management and claim appeals
Insurance eligibility and benefits verification
Accounts receivable management and collections follow-up
Specific services, scope, and pricing for any individual client are governed by a separate written service agreement between LabMed Billing and that client, which takes precedence over these Terms in the event of a conflict.

4. Client Responsibilities
Clients engaging LabMed Billing's services are responsible for:
Providing accurate, complete, and timely documentation, test orders, and patient information
Maintaining valid licensure, CLIA certification, and other regulatory credentials applicable to their laboratory
Reviewing and approving submissions where required under the applicable service agreement
Promptly notifying LabMed Billing of any changes to ownership, licensure, or payer enrollment status that may affect billing or credentialing
LabMed Billing is not responsible for denials, rejections, or compliance issues resulting from inaccurate or incomplete information provided by the client.
5. Fees and Payment
Fees for services are set forth in the applicable service agreement or statement of work. Except as otherwise agreed in writing, invoices are due within the payment terms specified in that agreement. Late payments may be subject to the terms outlined in the client's specific service agreement.
6. Confidentiality and HIPAA Compliance
Where LabMed Billing receives protected health information (PHI) in the course of providing services, that information is handled under the terms of a Business Associate Agreement (BAA) executed with the applicable client, in accordance with HIPAA and other applicable data protection laws. 7. Intellectual Property
All website content, including text, graphics, logos, and software, is the property of LabMed Billing or its licensors and is protected by applicable intellectual property laws. Clients may not reproduce, distribute, or create derivative works from this content without prior written consent, except as necessary for normal use of our services.
8. Limitation of Liability
To the fullest extent permitted by law, LabMed Billing shall not be liable for indirect, incidental, consequential, or special damages arising from the use of our website or services, including, but not limited to, lost revenue, denied claims, or delayed reimbursements resulting from payer decisions beyond our control. Our total liability for any claim arising from our services shall not exceed the fees paid by the client for the specific service giving rise to the claim during the 3 months preceding the claim.
9. Indemnification
Clients agree to indemnify and hold LabMed Billing harmless from claims, damages, or expenses arising from inaccurate information provided by the client, the client's failure to maintain required licensure or certifications, or the client's violation of applicable law.
10. Termination
Either party may terminate services in accordance with the applicable service agreement's termination provisions. Upon termination, LabMed Billing will cooperate in the orderly transition of client records and data, subject to applicable HIPAA retention obligations and the terms of the Business Associate Agreement.
11. Disclaimer of Warranties
Our website and services are provided "as is" without warranties of any kind, express or implied, except as expressly set forth in a separate written service agreement. We do not warrant that our website will be uninterrupted or error-free.
12. Governing Law
These Terms are governed by the laws of the State of Florida, without regard to conflict of laws principles, and by applicable federal law.
13. Changes to These Terms
We may update these Terms from time to time. Material changes will be reflected by updating the "Last Updated" date at the top of this page. Continued use of our website or services after changes are posted constitutes acceptance of the revised Terms.
